Job Description:

The Community Health Worker (CHW) is an integrative part of the Care Coordination team at MVH. The role collaborates to enhance the delivery of patient care services along the continuum of care. The CHW meets patient's needs efficiently and expeditiously by continuously improving the patient's experience, helping to ensure the institutional standards of high-quality patient care; reducing cost and ensuring reimbursement. The position requires a high degree of flexibility, independence, and willingness to participate in multiple activities and provide support to all members of the Care Coordination and the Physician offices. The CHW must have strong communication skills and the ability to converse comfortably with patients and their caregivers, practice staff and program staff and administration. The CHW will work with the Care Coordination team and the primary care team to ensure enrolled patients are receiving the services they need to achieve an optimum quality of life and to learn to manage their chronic illnesses.PERFORMANCE REQUIREMENT / DUTIES AND RESPONSIBILITIESResearch resources and programs available for patientsand their caregivers including but not limited to;Providing assistance for disabled and elder assistanceprograms;Coordinate legal, financial and insurance resources;Coordinate with Durable medical equipment contractors and resources for general home safety equipment;Supports systems for patients and caregiversexperiencing grief or bereavement;Provide health education material, including hard andelectronic storage system that is easily accessible forprogram staff.Assist Care Manager and Social Worker with follow up patient support callsContact members with memory/language issues toremind about appointments and medication issues; thisincludes editing, printing, mailing monthly calendars and appointment phone reminders for those specificmembers, as needed.Identify and refer to community services such astransportation, meals on wheels, day programs, andvolunteers.Coordinate referrals to internal and external resourcesincluding home care, Hospice, etc.Triage service requests and forward them to other program services, as necessary.Prepare and submit transportation applications and requests, as requested.Patient escort as needed.Refer patients to resources which would include,prescription drug programs, health and wellnessprograms, long term care insurance, health care proxy,asset and legal management, government programs,community agencies, public & private organizations,housing options and other services, as appropriate.Advocate for patients to ensure the members safety, physical, legal & financial well-being. Support their caregivers & quality of life goals and help navigate the healthcare delivery system.Work collaboratively with physicians, team nurses, careManagement team and the patients to support thePatient's plan of care by using motivational interviewing techniques.Supports patients who screen positive for SocialDeterminants of Health issues by managing the referral workqueue and helping patients with SDOH resources.Other projects and duties, as needed and specified by the Quality team and/or supervisor.Qualifications EDUCATION REQUIREMENTS Bachelor's degree in business or related field and/or sufficient work. Experience/performance to demonstrate comparable competency.
