As part of our multi-disciplinary team, the Nurse Practitioner/ PA
will provide initial evaluations, psychopharmacological management,
and behavioral management consultation. Typically, our NPs/PAs
travel to one facility per day. The population in each facility
varies among residents with dementia, depression, anxiety, or other
mental health diagnoses such as Bipolar Disorder and
Responsibilities of the Nurse Practitioner/ Physician assistant
include, but are not limited to:
- Assess current medical or psychiatric status.
- Review all current regular and prn medications, labs, x-rays
and any other pertinent data
- Monitor psychiatric medications for effectiveness, response of
resident, usage of prns and side effects. Inform residents of
potential side effects of psychiatric medications. Recommend (not
prescribe or order) the initiation of any psychiatric medication
- Monitor the tapering of psychiatric medications according to
federal regulations. Collaborate with the psychiatrist (and
attending MD as appropriate) to document when tapers are clinically
contraindicated for a resident. If a dose exceeds guidelines,
similar documentation will be done.
- Recommend nursing and, when appropriate, behavioral
interventions to nursing home staff. Develop behavior plans as
- Collaborate with nursing staff if a significant change in
status occurs and on an ongoing basis.
- Ensure that each psychiatric medication has an appropriate,
corresponding diagnosis. The Nurse Practitioner/PA will suggest
that all psychiatric diagnoses are updated on the doctor's
- Complete the AIMS testing every three to six months on
residents receiving an antipsychotic medication. Requirements:
Degree from an accredited college/university.
Proper Massachusetts licensure in order to fulfill position
Prior behavioral health experienced preferred. Experience working
with geriatric clients required.
Excellent communication skills.
Ability to build strong rapport with patients, families, and
nursing home staff.
