Responsibilities Implement PT treatment plans under the supervision and direction of the Physical Therapist. Deliver skilled PT interventions targeting mobility, strength, balance, and functional independence. Document patient progress and treatment notes accurately and within required timeframes. Collaborate with the interdisciplinary team including OT, SLP, and nursing staff. Educate patients and families on home exercise programmes and safe mobility strategies. Meet facility productivity standards while maintaining high-quality care. Requirements Current, unencumbered PTA licence/certification in New Hampshire required. Graduate of an accredited Physical Therapist Assistant programme. Minimum 1 year of SNF or post-acute rehab experience preferred. Current BLS/CPR certification.
