need it most within an innovative CCBHC model. What You’ll Do:
Provide essential medical screening and direct care services,
acting as a crucial link between behavioral and physical health for
our clients. Conduct medical screenings and provide direct care
services in collaboration with psychiatric providers, clinicians,
and primary care physicians. Work as a key team member within the
CCBHC to provide comprehensive, client-centered, evidence-based
care. Interface and coordinate with key partners involved in the
client’s care, including behavioral health professionals and social
services. Contribute to innovative approaches in behavioral and
population health management. What You Bring: Licensure: Current
Massachusetts license as a Licensed Practical Nurse (LPN). A
Bachelor’s in Nursing (BSN) is a plus. Experience: Behavioral
health and/or substance use disorder treatment experience is highly
preferred. Skills: Knowledge of addiction recovery, harm reduction,
or medication-assisted treatment (MAT). Sensitivity to cultural
diversity. Excellent communication and collaboration skills. Bonus:
Fluency in languages prevalent in Southeastern MA (e.g., Spanish,
Portuguese). Requirements: Ability to travel locally up to 30% of
