Summary:The Community Health Worker is a grant-funded position. The
Community Health Worker serves as a patient advocate and links
patients to services/resources, provides follow-up and encourages
positive behavioral changes. Responsibilities:Reasonable
accommodations may be made to enable individuals with disabilities
to perform the essential functions. Provides linkage to care for
patients, linkage is facilitated and strengthened retention efforts
are made to better maintain patients in care. Provide support for
chronic disease self-management to patients. Conducts initial and
periodic needs assessment, including assessing barriers and assets,
transportation, language literacy and cultural preferences. Promote
patient treatment adherence through assessing patient readiness to
make changes, assisting patients in making changes to daily
routines, assists patients in developing strategies to address
barriers. Coordinates access to basic determinants of health (food,
clothing shelter, utilities).Identify available community-based
resources and actively manage appropriate referrals, access,
engagement, follow-up and coordination of services. Attend and
participate in supervision, staff meetings, trainings, conferences,
and workshops to promote professional development. Document within
an integrated electronic medical record. Perform HIV/STI Counseling
and Testing for patients on a walk-in basis. Represent the health
center via outreach at community-based events. Perform case
management services for HIV and Hepatitis C patients. Serve as the
linkage to housing agencies, such as Steppingstone. The successful
candidate will:Attend Core requirement training for Community
Health workers and work towards Community Health worker
Certification.Attend Motivational Interviewing training and develop
competence in applying stages of change in all patient
interactionsWork with diverse patient populations and communicate
effectively in a culturally competent mannerBe an active member of
ID department multi-disciplinary treatment teamQualifications1.
High School graduate or GED2. Bilingual Spanish required (written
and oral fluency in a second language other than English
required)3. Good communications skills, organized4. Ability to
interact with high risk patients5. Sensitivity to HIV issues and
patients with disabilities required6. Reliable transportation
