    • As an LPN, you will identify health related needs and monitor the medical status of persons living in the home.

    • Provide direct care as needed, including ongoing health assessment, ADLs, transportation to & from appointments, and facilitating community outings.

        • Valid driver's license with clean driving record.
        • Must pass criminal background check.
        • Availability to attend required 3-day orientation (Details will be provided – some trainings may be virtual due to COVID-19)Physical Effort
          • Ability to stand, walk, bend, kneel, sit, reach, and climb stairs.
