A regional leading -healthcare -system located throughout the Cape region of Massachusetts is currently searching for an experienced RN Case Manager to join their award-winning -healthcare -team.Known as a regional leading provider of -healthcare -services, this organization combines nearly 500 Physicians and 5,500 employees across multiple hospital campuses, skilled nursing and rehabilitation facilities, and also a home health and hospice program. This organization strives to provide the highest quality of patient-centered care and to enhance the overall health and well-being of the surrounding communities.Reporting to the Director or Care Management, the RN Case Manager will lead the coordination of patient care by planning and implementing an individualized program of care that promotes quality and patient satisfaction. The RN Case Manager will work with varying patient populations amongst an interdisciplinary team. The RN Case Manager will utilized established standards and approved guidelines, works directly with third party representatives, and optimizes timely reimbursement.Requirements Include:

• Current Registered Nurse RN license in state of Massachusetts -
• Certified Case Manager preferred
