Because we have some of the most talented people in the world. # We are searching for an LPN # Home Care Liaison Coordinator to perform diversified duties including but not limited perform assessments of patients being referred to Home Care Services by the Case Coordinator; evaluate skilled services for home care within framework of health insurance reimbursement guidelines; investigate availability of resources and provide needed care, etc. Assist with education orientation of staff in regard to discharge planning and acceptance of patient issues as appropriate. Participate in all transition care initiatives to facilitate a safe discharge plan and prevent readmissions.# May provide functional or technical supervision of other assigned personnel where scope of supervisory responsibility is limited to occasional direction, guidance, explanation, etc. Hours: 40hrs (full time) Shift: Day Shift ##8:00am-4:30pm## w/weekend # holiday rotations Location: St. Luke#s Hospital New Bedford, MA # Education and/or Experience Requirements: Graduation from an accredited school of practical nursing or equivalent is required. Current LPN Licensure in the Commonwealth of Massachusetts is required. A minimum of five (5) years related work experience is required. A minimum of three (3) years current nursing experience in home health care is preferred.
