You will work with families in a hybrid of telehealth and in-person sessions to provide services. While working closely and learning from a team of specialists who are experts in childrens physical, cognitive, communication, social, emotional, and adaptive development. In addition to on-the-job learning, KDC offers professional development activities and training necessary to maintain relevant certifications and licensures.In this role, you'll get to:Partner with families to identify each childs strengths and developmental needsTogether develop an Individualized Family Service Plan (IFSP)Work closely with a transdisciplinary team on program planning and delivery of servicesOversee case management and referral services to assigned caseloadConduct home visits to provide developmental stimulation, parent training, and supportFacilitate parent/child and/or child groupsComplete reports of assessments, progress reports, IFSPs, and discharge summariesMaintain appropriate communication with physicians, clinics, and community agencies participating in each childs careWe are looking for people who have:Bachelors degree, or higher in Social Work requiredMust possess LCSW or LICSW requiredStrong written and verbal communicationExcellent organizational, interpersonal, and communication skillsA valid drivers license and reliable transportationThe physical requirements for this role:Frequent movement within classroom including but not limited to kneeling, bending, sitting, lifting, and positioning of studentsAbility to lift 50 pounds alone, 51+ pounds with the assistance of another person or adaptive equipmentDid someone say BENEFITS?!
